Please complete this form to submit your bill. All bills must be submitted by October 25, 2007. Any bill received after this date will cause the student to be dropped from the program. If you have any questions about or trouble with the form, please contact Ken Hussey at khussey@jcymca.org

You should write your bill in Microsoft Word or another program and save it before you submit your bill online. Be sure you have a saved copy of your bill before submitting this form. For information about how to write a bill and some examples, please review the Legislative Supporting Documents.
