Below are the forms and applications that are needed to register with Missouri YMCA Youth In Government. Please review each form and view the Key Dates and Deadlines for due dates.

| Student Registration Form Students must complete this online form to register for YIG. This form is due by September 30, 2010. |

| Student Participation Agreement Signature Page All students participating in YIG must sign this Signature Page, along with their parents or guardians, and return it to their delegation director. Students are required to review the Student Participation Agreement, listed below. Delegation Directors are encouraged to copy the signature page on the back of the Student Medical Form, thus only needing to send one sheet of paper per student to the State Office. |

| Scholarship Application Missouri YMCA Youth In Government will be offering scholarships ranging from $50 to $200 to offset program fees. Students interested in applying for a scholarship must complete this application and mail it to the State Office by September 30, 2010. |

| Adult Registration Form All adults volunteering with Missouri YMCA Youth In Government must complete the Adult Registration Form by October 14, 2010. |
