Below are the forms and applications that are needed to register with Missouri YMCA Youth In Government. Please review each form and view the Key Dates and Deadlines for due dates.

| Student Registration Form Students must complete this online form to register for YIG. This form is due by September 29, 2011. |

| Student Participation Agreement Signature Page All students participating in YIG must sign this Signature Page, along with their parents or guardians, and return it to their delegation director. Students are required to review the Student Participation Agreement, listed below. Delegation Directors are encouraged to copy the signature page on the back of the Student Medical Form, thus only needing to send one sheet of paper per student to the State Office. |

| Financial Assistance Application Missouri YMCA Youth In Government will be offering financial assistance, ranging from $50 to $175 to offset program fees. Students interested in applying for financial assistance must complete this application and mail it to the State Office by September 29, 2011. |

| Program Leadership Application Students interested in serving as a Supreme Court Judge or Legislative Program Leader need to complete this application by October 13, 2011. All applicants must review Student Leadership Overview before applying to review qualifications and duties of each position. |

| Candidate Registration Form Any student interested in running for one of the following offices must complete this online form: Attorney General, Governor, Lieutenant Governor, Secretary of State, and Speaker of the House. All candidates must also review the Elections and Campaigns documents. This form is due by October 13, 2011. |

| Candidate Agreement Students planning to run for Governor, Lieutenant Governor, Secretary of State, Attorney General, or Speaker of the House must complete the Candidate Agreement and mail it to the YIG State Office by October 13, 2011, not postmarked by this date. The agreement requires the signature of the student, parent/guardian, and delegation advisor. The Candidate Agreement can also be faxed to the State Office at (573) 635-0215. |

| Adult Registration Form All adults volunteering with Missouri YMCA Youth In Government must complete the Adult Registration Form by October 13, 2011. |

| Adult Medical Form All adults volunteering with YIG must complete the Adult Medical Form and return to their delegation director. The Adult Medical Form is due to the State Office by October 14, 2010. |

| Program Forms | |
| Bill Submission All Legislative participants must file a bill with the State Office by October 13, 2011. If two students are co-authoring a bill, only one form needs to be completed. Any bills submitted after the deadline will not be accepted and the student will be dropped from YIG. |
