Below are the forms and applications that are needed to register with Missouri YMCA Youth In Government. Please review each form and view the Key Dates and Deadlines for due dates.

| Student Registration Form Students must complete this online form to register for YIG. This form is due by September 30, 2009. |

| Student Participation Agreement All students participating in YIG must sign the Student Participation Agreement, along with their parents or guardians, and return it to their delegation director. |

| Student Medical Form All students participating in YIG must complete the Student Medical Form and have it signed by a parent or guardian and return it to their delegation director. |

| Scholarship Application Missouri YMCA Youth In Government will be offering scholarships ranging from $50 to $195 to offset program fees. Students interested in applying for a scholarship must complete this application and mail it to the State Office by September 30, 2009. |

| Program Leadership Application Students interested in serving as a Supreme Court Judge or Legislative Program Leader need to complete this application by October 14, 2009. All applicants must review Student Leadership Overview before applying to review qualifications and duties of each position. |

| Candidate Registration Form Any student interested in running for one of the following offices must complete this online form: Attorney General, Governor, Lieutenant Governor, Secretary of State, and Speaker of the House. All candidates must also review the Elections and Campaigns documents. This form is due by October 14, 2009. |

| Candidate Agreement Students planning to run for Governor, Lieutenant Governor, Secretary of State, Attorney General, or Speaker of the House must complete the Candidate Agreement and mail it to the YIG State Office by October 14, 2009, not postmarked by this date. The agreement requires the signature of the student, parent/guardian, and delegation advisor. The Candidate Agreement can also be faxed to the State Office at (573) 635-0215. |

| Adult Forms | |
| Adult Registration Form All adults volunteering with Missouri YMCA Youth In Government must complete the Adult Registration Form by October 14, 2009. |

| Program Forms | |
| Bill Submission All Legislative participants must file a bill with the State Office by October 14, 2009. If two students are co-authoring a bill, only one form needs to be completed. Any bills submitted after the deadline will not be accepted and the student will be dropped from YIG. |

| Judicial Trial Brief Form Each Judicial Trial Group must submit a trial brief with the State Office by October 30, 2009. Each team should email their brief as a Microsoft Word Document to Katy Lacy at klacy @jcymca.org. |

| Affiliation and Sponsorship Agreement This agreement is to be signed by the sponsoring YMCA or School. |
